Graduate from an accredited medical school
Hold a license to practice medicine in the state where residing
Complete at least one year of training in general surgery in an ACGME-accredited residency program
Complete a minimum of four years of training in otolaryngology in an ACGME-accredited residency program
Successfully pass a comprehensive written and oral examination
